How much do I pay for an out-of-network doctor at the ER?
The real answer
Across four carriers’ New Jersey plastic-surgery cells, ghosts out, the plan’s QPA sat at a median 0.74× the carriers’ median contract rate, below it on 20 of 26 codes; at 20 % coinsurance the patient’s share was a median $35 a code lower. Never any one carrier’s figure (on one of the four the QPA sits above its New Jersey median). Your doctor may vary; the law does not.

The fine print, in full
Standard Healthcare is a parody. No real company is depicted. Across four carriers' New Jersey plastic-surgery cells, ghosts out, the plan's QPA sat at a median 0.74× the carriers' median contract rate, below it on 20 of 26 codes; at 20% coinsurance the patient's share was a median $35 a code lower. Never any one carrier's figure (on one of the four the QPA sits above its New Jersey median). On 25 of that doctor's codes the QPA sits below Medicare's 2026 New Jersey schedule on 8 done in a hospital or surgery center and 14 done in an office — code by code, never the cell as a whole. By the stage of the procedure as CMS classes it, the same doctor's 20% share on the QPA is a band: $78 to $440 a procedure on a major procedure, the middle $186; $36 to $149 on the other procedures, the middle $77; the major stage higher on 92% of pairs, and the two bands overlap. Under the federal rule the recognized amount is the lesser of the doctor's billed charge and the QPA, what the plan calls its median in-network rate for the service (45 CFR 149.30, 149.140); where a state's own law reaches the plan, the state's amount. A dispute over the payment is the doctor's and the plan's; your share does not move (45 CFR 149.510).
CALL NOW!* *On many codes, one by one, never the cell. The label is in the fine print. Four carriers’ New Jersey cells. Your doctor may vary.
